Kaiser Permanente - Fall 2008 through Summer 2009

Kaiser Permanente is now the HMO option for eligible Fuller Pasadena Campus students who reside in Southern California, which covers preventative care along with injuries and illnesses. 

In addition to rates that are significantly lower then our previous HMO our Kaiser plan also offers some additional benefits including chiropractic care. 

Students who are required to have health insurance are eligible to purchase this insurance.

If you are having trouble enrolling online or the online enrollment process is not available to you, you may also print and fill out the above form and turn it in to the Student Health Insurance office by the quarterly deadline.*


*This form should be submitted in the Fall quarter or the first quarter that a student purchases Kaiser during the academic year, whichever comes first.  After printing the document, please complete the form and verify that your information is complete.  Please be sure to sign the form.  You may fax your completed form to the Student Health Insurance office at 626-204-2072.


 Health Insurance Cancellation Form (PDF)

  • This form is for students who meet the eligibility criteria for cancelling their medical and/or dental insurance by the quarterly deadline for the applicable quarter.

  • To waive insurance if you have coverage that meets the minimum requirements, go online to www.renstudent.com/fuller to complete the process.

  • Submit this form and proper documentation to the Student Health Insurance Office by the applicable quarterly deadline.
