Voluntary Student Dental - Fall 2008 through Summer 2009

Fuller Theological Seminary offers a dental program for students through United Concordia Insurance Company. Students (including those at extension sites) residing in California and enrolling in one or more units in a quarter are eligible to sign up for this insurance on a voluntary basis. Please refer to the plan documents for more eligibility information.

Deadlines:

The deadline to purchase dental insurance is the first Friday of each quarter.

The preferred quarterly deadlines are:

Fall - 09/05/08, Winter - 12/05/08, Spring - 03/06/09, Summer - 06/05/09

The final deadlines for each quarter are: 

Fall - 10/03/08, Winter - 01/09/09, Spring - 04/03/09, Summer - 06/26/09

  • Dental Providers (PDF)
    You may locate dental providers on the United Concordia's web site. However, if already enrolled, please be sure to obtain treatment from the dentist office you have already selected. Or, contact the Student Health Insurance Office to request to change your dental provider.
     
  • Dental Privacy Notice
    You may access United Concordia's Privacy Notice by clicking on the Dental Privacy Notice link which provides information on the availability of the HIPAA Privacy Notice and ways to obtain the notice.
     
  • Davis Vision Discount Program
    This year the United Concordia Dental Insurance coverage includes a vision discount program.  Click on the link above to see what benefits are provided.

Dental Insurance Forms:

  • Health And Dental Continuation Form (PDF)
     This form is only for those who are continuing insurance coverage one quarter beyond graduation or being enrolled in classes (payment must be included with form).
     
  • United Concordia Dental Enrollment Form (PDF)
    This form should be completed and submitted in the Fall quarter or the first quarter that a student purchases dental insurance, whichever comes first. Voluntary Student Dental Enrollment Kits are also available in the Student Health Insurance Office.
  • Health/Dental Insurance Cancellation Form (PDF)
     

    • This form is for students who meet the eligibility criteria for cancelling their medical and/or dental insurance by the quarterly deadline for the applicable deadline.

    • Submit this form and proper documentation to the Student Health Insurance Office by the applicable quarterly deadline.
