PRACTICUM SITE
DAY
Practicum Site Day
is conducted during the Winter Quarter, first Friday of the first week
of class, from 9 a.m. to 12 noon. This allows prospective practicum
students an opportunity to meet with representatives of all approved
practicum sites.
Each director and/or
representative makes a presentation of her/his clinic as well as
participates in an informal question and answer time. At the time of
this meeting, students should prepare a resume in the event one is
requested from the site representative.
For those students
postponing practicum, interviews may be arranged with practicum sites
throughout the year.
THE APPLICATION
PROCESS
Students request
interviews with the sites at which they intend to apply. The agency will
determine which student(s) they will interview and will set up interview
times. It is the student’s responsibility to arrange for these
interviews. Final decisions regarding practicum placement are typically
made during Winter Quarter. Students will be notified by the end of the
ninth week of the quarter as to their status.
THE PRACTICUM
AGREEMENT
Once the agency has
made its selection, and trainees have accepted, the Practicum Agreement must be signed and returned to the Clinical Coordinator.
This can be completed during the agency’s first week of orientation. In
all cases these forms must be completed and submitted
prior to beginning practicum. This is particularly important
for agency insurance purposes. This completes the practicum selection
process.

Qualification
This form should be submitted only if
the site has not been approved by the Director of
Clinical Training. Check with the Clinical Coordintor.

Submit a brochure or any printed materials on the practicum site,
intake forms, etc.

Supervisor Qualification
& Vitae
A supervisor qualification form along
with a vitae must be submitted for every supervisor that will be
supervising the trainee and signing off hours.

Completed after approval has been made by the DCT.
